Introducing My UKHealthCare. Online access to your health records.

UK HealthCare is pleased to offer online access to your health records* via the My UKHealthCare patient portal.

This offers you a convenient, secure way to access your records and connect with UK HealthCare through self-service online tools.

Once you register for the My UKHealthCare patient portal, you can:

  • Request prescription renewals when it’s convenient for you, not just when our offices are open.
  • Request, cancel or reschedule appointments.
  • View lab test results, radiology reports and office visit summaries.
  • Get health maintenance reminders.
  • View your immunization record and allergies.
  • View your hospital discharge instructions.

 You can also use the portal to send your records to another provider using secure email.

*Note that records available through the portal are not necessarily historically complete. Records included extend as far back in time as the electronic health record used by the particular clinic, usually 1-2 years.

To request your complete UK HealthCare record, follow the directions below.

For a printed copy of your complete UK HealthCare medical records:

  • The Health Information Management (HIM) department offices can usually respond to requests for medical records within 30 days. The average turnaround time is 2-4 weeks.
  • Records cannot be provided immediately or "on the spot" due to the time it takes to gather the requested information and process the request.
  • Medical records may be mailed to you, or you may make an appointment to pick them up in the HIM department.
  • Medical records may be faxed to your physician.

You can get printed copies of your records in the following ways:

  • Your doctor can request your records.
    The doctor's office can call 859-323-5117 to request your medical records. We will fax the requested records directly to the clinic the day before or the day of the appointment. This is the quickest way for your doctor to receive your medical records.

  • You can call for the form to get your records.
    Call us at 859-323-5117. An authorization form and a pre-addressed, stamped envelope will be sent to your address. Complete the authorization form, then either mail back or fax the form to the medical records department at 859-323-6853 or 859-257-1347.

  • Print the authorization form and mail or fax it.

    Either mail the form to the Health Information Management office listed below where you received treatment or fax the form to 859-323-6853 or 859-257-1347.

  • If you are at UK A.B Chandler Hospital...
    You may visit the Health Information Management Department in Room C106 on the first floor of UK Chandler Hospital to fill out the authorization form.

If you have been a patient at more than one location, send your form to our Health Information Management department office at the UK Chandler Hospital address and we will gather your records from each location.

Costs

The first copy of your records is free. A standard fee of $1.00 per page will be charged for additional copies. Please keep a personal copy of any information you request to avoid paying for your records again.

At your request, your medical records will be released to any doctor free of charge.
